Sample Survey Opening Page
Note.
When creating a survey, remember to include an opening page, at least one body page, and a closing page. In the opening page, include your name and contact information as the researcher, the survey's purpose, the name and contact information of your research supervisor, your intended audience, and the appropriate privacy notices and methods for reporting the survey's results.
